CONSUMER INFORMATION

PLEASE READ THIS INFORMATION FOR CONSUMERS IN THE STATE OF TEXAS CAREFULLY AND IN ITS ENTIRETY.

House Bill 4224 (89th Regular Session), which went into effect on Sept. 1, 2025, requires licensees to, per Section 181.105 of the Health and Safety Code, prominently post on their website and at any facility detailed instructions for a consumer to:

  • Method of which consumers can request the health care records from the licensee:

    • ​All records requests must be made in writing and sent to admin@legacytrailstherapy.com or via mail (USPS, UPS, FedEx) to the primary practice office:

      • Legacy Trails Therapy, PLLC5300 Town & Country Blvd., Suite 240, Frisco, TX 75034.

      • If there is a joint file, all parties MUST sign a release of information for records to be released.

      • Electronic records will be shared through the practice's secure portal.

      • Fees apply.

  • The Texas Behavioral Health Executive Council can be contacted through various modalities:

    • Complaints can also be submitted through the Office of the Attorney General, file a consumer complaint
