You will receive an email from our EHR portal (Sessions Health) to set up your file. Below is a list of items to provide so the first session can be conducted smoothly and in a timely manner: Driver's license or government ID ALL Intake paperwork completed Release of Information for any healthcare providers, family members, or support people you would like to have access to your personal information and/or medical records. (This form is available in your portal) A credit card on file that the bank/financial institution permits to be "vaulted" and charged at the time of service. Current health insurance card (if you are using insurance) Any existing custody-related documents (i.e., court-related documents such as a divorce decree, co-parenting plan, etc.) and/or relevant court orders for minors or mandatory court-ordered therapy services.   If you have any questions about any of the items needed for your initial appointment, please email us: admin@legacytrailstherapy.com

We value your time and strive to reserve your scheduled therapeutic hour just for you. If you are unable to attend your session, please let us know as soon as possible. Sometimes clients seek same-day sessions and would benefit from using a pre-canceled spot. If that day/time slot no longer works for you, please let us know, and we can find a different day/time.    Communicating with us is essential. If you need to cancel, for whatever reason, we require at least 24 hours' notice. A $100 fee applies to cancellations made within the 24-hour window before the session start time and to no-shows. Please note that insurance will not cover this cancellation fee. An appointment is considered a no-show if the client is 10+ minutes late. In this case, the therapist will forfeit the session and reach out to attempt rescheduling for another day/time. Please contact us with any questions!
